Waitlist privacy

The founding waitlist collects your name and email address so Duval DPC can contact you about the clinic’s opening and membership details. We store those details for launch planning and do not ask for medical information or payment.

Joining the list does not establish a patient relationship, reserve care, or enroll you in a paid membership. Please do not send symptoms, diagnoses, or other health details through the form.

To request removal from the waitlist, email duvaldpc@gmail.com. This page will be updated when the full practice privacy notice is ready.

Contact messages

The general contact form sends your name, email address, and message to Duval DPC using Resend. We use this information to answer your inquiry. We keep a limited submission log to prevent spam.
